Assisted Reproductive Technologies (ART)
- In Vitro Fertilization (IVF): IVF is an assisted reproductive technology that involves fertilizing eggs with sperm in a laboratory. The process involves:
- Egg Retrieval: A minor procedure where eggs are gently aspirated from the ovaries using a thin needle. The retrieved eggs are then taken to the lab for fertilization.
- Fertilization: In the lab, eggs are fertilized with sperm.
- Embryo Transfer: The fertilized embryo is then placed into the uterus, where it can implant and lead to pregnancy. IVF is often recommended for couples with endometriosis, low sperm count, blocked fallopian tubes, or unexplained infertility.
- Intracytoplasmic Sperm Injection (ICSI): This is a specialized type of IVF where a single, live sperm is directly injected into the center of an egg. This technique increases the chances of fertilization and is often used in cases of severe male factor infertility.
- Assisted Hatching: This technique is a crucial part of the IVF process. We create a small opening in the embryo’s outer shell (zona pellucida) to help it “hatch” and attach to the uterine lining, which is essential for a successful pregnancy.
Intrauterine Insemination (IUI)
- Intrauterine Insemination (IUI): As a less invasive option than IVF, IUI involves placing concentrated and washed sperm directly into the uterus around the time of ovulation. The goal is for the sperm to swim into the fallopian tube to fertilize an egg, leading to pregnancy. This is often recommended for individuals with mild male factor infertility or cervical issues.
Sperm Retrieval Procedures
- Percutaneous Epididymal Sperm Aspiration (PESA): A minimally invasive procedure that uses a fine needle to retrieve sperm directly from the epididymis. This is typically used for cases of obstructive azoospermia resulting from a past infection or a vasectomy.
- Microsurgical Epididymal Sperm Aspiration (MESA): Similar to PESA, MESA is a procedure that collects sperm from the epididymis using a thin needle. The collected sperm can be cryopreserved for future use.
- Testicular Sperm Aspiration (TESA): This minimally invasive surgical procedure involves inserting a needle directly into the testicle to extract sperm.
- Microdissection Testicular Sperm Extraction (Microdissection TESE): Unlike TESA, TESE involves the removal of a small tissue sample from the testicle, which is then examined under a microscope to identify and extract viable sperm. This method is often used for cases of non-obstructive azoospermia.
